Adult Plan
For ages 13 and older

This plan is ideal for adults who need regular cleanings and have no current periodontal disease. It includes:
- 2 Professional dental cleanings per year
- 2 Doctor exams per year
- Necessary X-rays*
- One emergency exam per year (if needed)

15% Off all other treatments (some exclusions may apply)
Periodontal Plan
For patients with periodontal (gum) disease
$62.00/month
$99.00 Lifetime Activation Fee

This plan provides extra care for patients with ongoing gum health concerns. Benefits include:
- 3 to 4 Perio maintenance cleanings per year
- 2 Doctor exams per year
- Necessary X-rays*
